Informative personal record of pension insurance
Citizens may ask the Czech Social Security Administration to provide an extract of insurance periods from the CSSA register, namely to provide an informative personal record of pension insurance (the so-called IOLDP). This informative record shall contain a summary of pension insurance periods and, where appropriate, periods treated as insurance periods, kept in the records of CSSA.
Submission of a request for informative personal record of pension insurance (IOLDP)
Citizens may apply for an informative personal record of pension insurance (hereinafter the informative record):

service Request to generate an informative personal record of pension insurance (This service can be used by holders of data boxes, eObčanka (an electronic ID card with a chip) or user accounts (name, password, SMS code will be activated at Identita občana)

by completing and returning the form Request to send an informative personal record of pension insurance (The form may also be printed and sent via the Czech Post)

The request must include the Czech birth number of the applicant, first name and surname and address or data box ID where the informative record is to be sent.
The citizen has a right to have the informative record sent annually. The informative record shall be sent by CSSA to the citizen within 90 days of the date when CSSA received his/her request (Section 40a of Act No. 582/1991 Coll.).
The missing period of insurance is not examined by the Czech Social Security Administration for the purposes of the informative personal record of pension insurance . For more information on how to demonstrate the absence of insurance periods (employment) and on conditions of the pension entitlement seek the help of DSSA, or the client centre at the Prague Headquarters.
